A piercing in the foreskin. Not available to victims of circumcision. However, in many cases of circumcisions that are done "well" (please note my qualification of that term), there is still a usable, and pierceable portion left around the "neck". Add to the fact that the male genitals are greatly desensitesed from circumcision, and it just could work. If you're in that state of mind.
But, oh my god, ouch.
